Safe Transitions provides transportation and assistance with medical appointments, assistance with personal needs shopping/budgeting, teaches independent living skills, and coping skills for symptoms of mental illness. We get residents involved in normal activities to improve their quality of life, such as fishing, bar-be-ques, scenic railroad excursions, Como Park, and the Minnesota State Fair. We have residents with mental illnesses, TBI, and CD issues. Region 7E has preference for referrals. Safe Transitions must screen all potential clients and also consider the referral that seems the best fit taking into account the other clients in the home.

The house is located in a pleasant neighborhood with nearby shopping stores. Safe Transitions provides transportation and assistance with medical appointments, assistance with personal needs shopping/budgeting, teaches independent living skills, and coping skills for symptoms of mental illness. We get residents involved in normal activities to improve their quality of life, such as fishing, bar-be-ques, scenic railroad excursions, Como Park, and the Minnesota State Fair.
