ADULT FOSTER CARE
Safe Transitions currently provides services in 11 adult foster care homes throughout Minnesota.  Each home offers an individualized approach to treatment and support for people with mental illness. Services offered include medication management, medication education, medication supervision, physical health management, independent living skills building, transportation, trained staff supervision (24 hours at most homes), groups, 1x1 counseling, socialization opportunities, and much more, in a home environment.

COMMUNITY SERVICES
Safe Transitions' Independent Living Program (ILP) offers people the option of bringing our services to their own home. Home visits are offered as often as daily, depending on individual needs.

ADULT REHABILITATIVE MENTAL HEALTH SERVICES (ARMHS)
Our newest service available to adults.  Adult Rehabilitative Mental Health Services focus on helping a person to achieve goals which their mental health interferes with.  ARMHS is for individuals who could benefit form services to regain skills related to independent living or managing their mental health.

PERSONAL CARE SERVICES (PCA)

Our PCA program specializes in assistance for people with mental health/behavioral issues and other disabilities who live or their own homes.

INDEPENDENT LIVING SKILLS COUNSELING (ILSC)
Our counselors provide quality, hands-on, skills training based on each person’s individual needs. Skills training areas include cooking, cleaning, laundry, grocery shopping, money management, personal care, and other life management skills.

GROUPS
Group topics include social skills, independent living skills, healthy alternatives, medication education, and assertiveness.

TRANSPORTATION
Transportation to medical appointments, grocery shopping, groups, and social events is offered to Safe Transitions’ clients.

MENTAL HEALTH PROFESSIONAL & RN SERVICES
Safe Transitions is proud to have professional oversight of all our programs.  Professionals provide supervision to the Safe Transitions’ direct service staff and services to recipients of the programs. They also offer one to one counseling to people in the program, group facilitation, and staff training.

VOCATIONAL ASSISTANCE
Individualized assistance is provided in Safe Transitions’ programs for people to achieve their vocational goals. From basic day to day job skills training, to transportation to work, to job counseling, Safe Transitions helps to keep people on the right path to being vocationally successful to the best of their ability.

CHILDREN'S SERVICES
Our children's programs provide PCA and Waivered services to children in the community.   Services are developed specifically for each child’s/family’s needs.
