    Safe Transitions currently has a part-time Individual Home Supports (IHS) positions available in Isanti County.

    The Individualized Home Supports (IHS) assists adults with mental illness to live independently by teaching life skills (social skills, budgeting, meal planning, grocery shopping, etc.), accompany to medical appointments, and med monitoring. The ILS Counselor is responsible for the monitoring of client behaviors and program services to assure the safety and well-being of each client.

    Flexible hours.
    Experience working with the general public and/or with the aging population helpful but not necessary; training provided. Company vehicle available, or mileage reimbursement provided.
    A valid driver’s license, state and county background clearance is required.
